It is important to us that you know your rights as a client. Some of this information is provided to you in your informed consent form, as well as discussed in sessions. Requesting Records
If you would like to request records from your health care provider, please request them in writing to info@familytablecounseling.com.
Please specify which records you will need, specific dates, and name(s) of client(s), and relationship to client(s).
Your health care provider may contact you for further information regarding records requests to ensure compliance with privacy and possibly to request a release of information, depending who receives the records. Also, please note that administrate fees may apply for some records preparation.

Good Faith Estimates
As of January 1, 2022, the “No Surprises Act” protects uninsured and self-pay clients from many unexpectedly high medical bills. If an individual does not have, or plans not to use, their medical insurance, they are entitled to a "Good Faith Estimate" which details the prices of services and goods they expect to receive under the care of their healthcare provider.
